This would be the registered owner or managing agent. • Birth certificate for each household member (passport, I-94, naturalization certificate, or resident alien card may be supplied for non-US born applicants). • Government issued picture Id for each adult household member (State Id, Driver’s License…). • Social Security cards for each household member. • 6 months of the most current consecutive bank statements from all financial institutions confirming the balances in your checking, savings, money market, and certificates of deposit accounts; stocks, bonds and real property tax receipts. • Employment paystubs (last 6 months), verification of social security (award letters from social security dated within 90 days), public assistance benefits and any other sources of income. • A copy of your most recent Federal Tax Return (1040 or 1040A) Include all W-2 Forms.
